Our Mentoring Process:
Individualized Mentorship Plan
During the first meeting, the individual and mentor will develop an individualized mentorship plan. This plan will identify the specific needs of that individual including but not limited to support with their living situation, organizational skills, development of a self-care plan, execution of therapeutic skills, and maintaining sustainable recovery. The mentor will then facilitate the identification and development of reachable short term and long term goals. These goals will take into consideration each individual’s values, passions, mental health struggles, and family dynamics. The therapeutic mentorship plan is reviewed on a weekly basis and will allow both mentors and individuals to track progress toward achieving set goals while ensuring optimal support. This process also supports individuals with increasing and maintaining motivation to move forward.
Weekly Meeting(s)
Each week, the client and the therapeutic mentor will have one hour long meetings to go over weekly goals, to-do lists, appointments, etc. These meetings can happen at our office in Westlake Village or out in the community at a coffee shop, 12-step meeting, or the client’s home. These meetings can be altered based upon client’s specific needs each week. Additional weekly meetings may be added based upon a client’s needs.
Calendars + Journals
At Bridge The Gap, we pride ourselves in providing clients with tools that they can utilize even after they’ve graduated from working with a therapeutic mentor. Mentors help their clients develop an effective system of organization which often involves day planners and journals. This day calendar will be utilized in the client and therapeutic mentor’s weekly meetings. The planner with be used to plan client’s weekly appointments and commitments and help them stay accountable with the therapeutic mentor’s support. The journal will provide client’s a singular space to keep track of their goals and tasks for each week. In the following meeting with client’s therapeutic mentor, they will be able to go over both, client’s calendar and journal.
Daily Check-Ins
Monday through Friday, clients will have both an evening and morning check-in with their therapeutic mentor. In the morning, clients discuss plans and goals for that day. In the evening, the client and therapeutic mentor will reflect back on the day.
Transitioning
Our experience has shown us that to successfully launch and empower individuals to be on their own, a transitional process is critical. At Bridge The Gap, once an individual is ready to transition from working with a mentor to being on their own, we implement a taper down transitional process. The mentor, support team, and individual that we are mentoring will collaborate to create an appropriate plan to tapper out of the mentoring support in a way that empowers that individual to move forward with confidence while maintaining their recovery and self-care.
Consulting Services
Navigating the therapeutic world can often times feel overwhelming, especially when emotions are invested in the process. Bridge The Gap mentors provide consulting support to individuals and families as they choose the appropriate level of care for their loved one.
Bridge The Gap’s core value is collaboration. Often times, individuals and their families enter upon the mentorship relationship in need of additional supportive resources. Our team and founders work effortlessly to build an abundance of excellent resources to be able to make appropriate referrals to those we work with. We make continuous efforts to expand our local network. These collaborative resources include but are not limited to specialized therapist, educational consultants, psychiatrists, schools, physicians, pediatricians, neurofeedback, residential treatment programs, partial hospitalization programs, intensive outpatient programs, community resources, the list goes on.

At Bridge The Gap, our mentors believe in supporting and facilitating the development of skills that provide individuals with a foundation to flourish and succeed. Development of such skills allow individuals to maintain a clear and open headspace while engaging in a journey of emotional growth.
