Collaboration with Families

For families, the therapeutic mentor will act as a liaison between the support team, your loved one, and you. For therapists, the mentor will meet with clients in real world settings to put therapy homework into action and take case management out of the therapy room. For treatment teams, the mentor will provide support in transitioning from a higher level of care to a lower level of care while acting as the point of contact between the individual and their treatment team. We bridge any and all gaps.

 


Weekly Communication

Bridge The Gap Services maintains communication with the client’s parents, family members, and/or loved ones with weekly phone conversations. These phone conversations provide families with an opportunity to voice concerns, strengths, and/or areas for growth regarding the client. Additionally, the therapeutic mentor will request to be in weekly contact with client’s treatment team to maintain cohesive care. Ongoing communication with family members is imperative for reparation of the family system.

Weekly Email Updates

Bride The Gap Services provides parents, family members, and/or loved ones with weekly email updates. Each week, an email is sent to the client’s treatment team and family members to update everyone involved on client’s week – what client accomplished that week, client’s goals and plans for the following week, and areas for improvement.


Family Support Group

Bridge The Gap Services offers a biweekly support group in Agoura Hills for all family members of clients current and past. Family members are encouraged to participate in their own self-care and therapeutic support, working alongside their loved ones they worry so much about.

When clients seek help, the family unit is often struggling and stressed, feeling hopeless about their ability to have a “normal family life”. It is important for the family members to have a place to come, get support, guidance and education about codependency issues, setting boundaries, self-care and taking the focus off of their loved one who has become the focus and putting it back onto the family’s individual members. In our weekly support group family members will find a safe, confidential environment to talk, ask questions and get guidance. Family members who have been in the trenches in the past, and are now in a healthier place are welcome to come and provide feedback and support as a way of paying it forward and being of service.

The support group is held the 1st, 3rd, & 5th Tuesdays of each month virtually via zoom. Please reach out for more information or to participate.
